How Ksharsutra Works for Fistula

The fistula tract is first identified and its full length assessed. A specially prepared Ayurvedic medicated thread is then passed through the tract, tied loosely, and left in place for one week.

Over the week, the alkaline medications on the thread:

  1. Debride necrotic and unhealthy tissue lining the tract
  2. Stimulate healthy granulation from the base
  3. Progressively narrow the tract wall through controlled tissue action

At each weekly visit, the thread is tightened or replaced. Over successive weeks, the tract shortens and heals from the inside out. When the tract has fully healed and the thread cuts through, treatment is complete.

The sphincter muscles, which the fistula passes through or near, are preserved throughout because the gradual controlled action does not cause the abrupt injury of surgical division.

Ksharsutra vs Fistulotomy

Conventional fistulotomy involves cutting open the fistula tract, dividing whatever tissue lies in the way including sphincter muscle. This works well for superficial fistulas but carries significant incontinence risk for high or complex fistulas.

Ksharsutra preserves sphincter integrity by progressive gradual action instead of one-time cutting. This is particularly valuable for:

  • Transsphincteric fistula
  • Suprasphincteric fistula
  • Multiple-tract complex fistula
  • Recurrent fistula after previous surgery

The IFTAK Protocol (Interception of Fistulous Tract & Ksharsutra)

What is the IFTAK Technique?

IFTAK (Interception of Fistula Tract And Ksharsutra) is an advanced, specialized hybrid para-surgical technique developed specifically for complex, high transsphincteric, and recurrent anal fistulas.

In classical Ksharsutra therapy, the medicated thread traverses the entire length of the fistula tract from the external skin opening to the internal mucosal opening. While safe and sphincter-preserving, extensive or long tracts can require 8 to 16 weeks of weekly thread replacements.

How the IFTAK Technique Works

  1. Interception of the Lateral Tract: Under local or regional anaesthesia, the peripheral, non-sphincter portion of the fistula tract (between the external opening and the outer border of the external anal sphincter) is surgically excised or debrided.
  2. Ksharsutra Application to the Sphincter Segment: The classical medicated Ksharsutra thread is placed exclusively through the proximal segment that passes through the anal sphincter muscles into the internal opening.
  3. Dual Healing Mechanism: The external lateral wound is allowed to heal by secondary intention (granulation) within 2 to 3 weeks, while the Ksharsutra thread performs its progressive debridement, chemical cauterization, and sphincter-sparing cut-through on the critical deep segment.

Key Clinical Benefits

  • 30% to 50% Faster Total Recovery: Substantially reduces the number of weeks required for complete tract clearance compared to classical thread cut-through of long tracts.
  • Ideal for Travelling Patients: Greatly reduces the required frequency and duration of weekly hospital visits—especially beneficial for patients travelling from a distance where weekly visits over months are impractical.
  • Absolute Sphincter Preservation: Avoids any abrupt incision through the muscular continence rings, maintaining the gold-standard safety profile of classical Ayurveda.
  • Proven in Recurrent Cases: Particularly valuable when prior conventional surgeries (fistulotomy, flap repair) have failed and created extensive scarring.

Success Rates and Realistic Expectations

Ksharsutra has high success rates in appropriately selected fistula cases when performed by experienced practitioners. Complete healing is expected in the majority of cases. Recurrence rates are comparable to or lower than surgical alternatives, particularly for complex cases.

We are honest about limits:

  • Very complex cases may require staged treatment
  • Some cases benefit from initial imaging before Ksharsutra begins
  • Underlying conditions (Crohn's disease, tuberculosis, diabetes) affect outcomes and must be addressed alongside
  • No technique offers 100% success

Case Selection

Not every fistula is best treated with Ksharsutra. Our consultation includes:

  • Detailed history including any previous treatments
  • Examination
  • Imaging (MRI fistulogram) for complex or recurrent cases
  • Assessment of underlying conditions
  • Discussion of all appropriate options (Ksharsutra, laser, conventional surgery)

We recommend Ksharsutra when it is genuinely the best option for your case. If another approach fits better, we recommend that.

What to Expect

First Visit: consultation, examination, discussion of options, preoperative evaluation.

Ksharsutra Application: performed under local anaesthesia in the OT, typically 20 to 45 minutes, same-day discharge.

Weekly Sessions: brief OPD visit, thread tightened or replaced, progress assessed.

Completion: confirmed healing, follow-up at 3 months and 6 months to detect any recurrence early.
